What you have is what looks to be a "cross" between these two styles of vintage doctor bag:

She has a pretty good guide to fake Gucci and she has tons of pictures of real and fake ones.
If you compare the pictures to the one you have, you'll start to notice differences between them. Your bag isn't really exactly like one or the other.
Alignment simply means that ALL designers (whether Gucci, Chanel, LV, Coach) like to have their patterns perfectly lined up and centered. It's kind of hard to see how they have it lined up in the picture I provided... but the pattern is centered with the center of the bag and it is one continuous piece of fabric all the way across. Your bag, the pattern is shifted off to one side a bit and it looks like the pattern isn't lining up vertically on each side of the leather pieces.
